In the box labeled Well-Known Applications is a drop down menu, choose the option of User Defined.

Next comes the Port Range. If you are forwarding only a single port then enter that number into both the Port Range and Local Port boxes. If you are forwarding more than one, then enter the range in both of these boxes. It should be in the format of lowest number, dash, highest number such as 22-26.

In the Local IP box, enter the IP Address that you are forwarding these ports to. For instance, if you are forwarding ports to run a program on your computer, then the IP address you enter here needs to be that computer's IP Address.

Under the drop down menu of Protocol choose the type of protocol you want to use for the ports you are forwarding.

In the last box on the right is the Description. Enter the name of the program here or something that helps you remember why you are forwarding these ports.
