You are now on a page like the one you see here. In the first row of empty boxes, place a check in the box labeled Enable.

To the right of this is the box tagged Description. Enter the name of the program here, or something that is going to remind you why you are forwarding these ports.

If you are forwarding a single port, enter that port number into the boxes labeled Inbound port and Private port. If you are forwarding a range of ports, enter this range in both of these boxes. Enter them like 500-600, with the lowest number first, highest number last, separated by a dash.

Next is a drop down menu with the label of Type. This is the protocol type. Use this menu to choose the type of protocol you are going to be using when you forward these ports.

The last box you have is the Private IP address. Enter the address you are forwarding these ports to in this box, for example if you are forwarding ports to run a program on a computer, you need to use that computer's IP address.

That's it, you're done. Just click Add and Apply Changes before you leave this page.
